APPOINTMENT INFORMATION
Your first appointment
For your first appointment, we ask that you bring a photo ID and your insurance card. We also ask that you arrive 10 minutes early to complete any necessary forms or to update your information. Please note, we do our best to see you as close to your scheduled appointment time as possible, but due to the nature of our business, our providers may be delayed (babies don't always tell us when they are coming)!
Insurance card
It is important to bring your insurance card to every appointment to verify changes have not been made, as well as to make sure we continue to participate with your current insurance plan.
Running late?
As a courtesy to the other patients waiting to see your provider, please reschedule your appointment if you will be late by 5 minutes or more.
Need to cancel an appointment?
You can easily cancel an appointment by calling or texting us at 920.729.7105. We understand life happens and sometimes you just need to cancel. That's okay! We simply ask that you at least give us a head's up. Not showing up without a forewarning disrupts the provider's schedule and the unfilled slot is a lost chance for another patient to be seen. We respect that your time is valuable too and appreciate your understanding and cooperation.
