Insurance and Billing
As a courtesy to you, we accept most insurance plans and will submit your claims accordingly. However, we encourage you to verify your benefits with your insurance company prior to your appointment. This will ensure you'll receive the maximum benefits available to you.
Your plan may require that you pay a copayment*, a deductible, or part of the bill (coinsurance). If you have any questions about your financial responsibilities or benefits, you should contact your Human Resources Department or call the Customer Service number listed on your medical insurance card.
Please bring your insurance card to each visit. A sample insurance card is below highlighting the important information you need to know for your office visit. Please keep in mind that all insurance cards look different so the information may be in a different location on your card.

Statements
You will receive a monthly statement listing charges for services
received, pending insurance claims, and any payments made in the last 30
days. Your financial responsibility will be listed in the patient
balance column and payment in full is required by the date due.

We do accept CareCredit® as a form of payment for our clinic.
CareCredit is a healthcare credit card that can be used as a payment
option for certain expenses not covered by insurance or to bridge
situations when desired care exceeds insurance coverage. Please visit
the CareCredit website for additional information.

*Copayments are due at the time of your office visit.
