Women's Care is open to new OB and GYN patients. Appointment times vary by provider. Please call the main office number to schedule an appointment at either office.
We try to see you as close to your scheduled appointment time as possible and ask that you arrive 10-15 minutes early to complete any necessary forms or to update information.
If you decide you don't need your appointment, please call the office as soon as possible to cancel your appointment. Not showing up disrupts your provider's schedule and the unfilled slot is a lost chance for another patient to be helped.

New Patients
When you schedule your first appointment (non-emergent) the receptionist will mail forms below to you to complete and return prior to your appointment date.

Established Patients
You will be asked to complete a short medical history update form at your yearly exam or if you have not been in for an appointment for awhile. To save time in the office you may want to print the "Appointment Questionnaire" form above and complete it prior to your appointment and bring it with you.
If you are transferring your care from another physician or want your OB/GYN provider to review your medical information from another source, you need to complete a release form. The completed form is submitted to the provider that has the records. They may ask you to pick up the information or they may send it directly to our office. There should not be a charge for copying medical records that are part of your treatment by another provider.

Running late? As a courtesy to the other patients waiting to see your provider, please reschedule your appointment if you will be late by 10 or more minutes.
Need to cancel an appointment? Please try to call as early in the morning as possible so the appointment time can be used for someone on the waiting list.
